Mater HREC application requirements

Please read the instructions below carefully before submitting your application to the Mater Research Ethics Office. Any submissions that are not complete are considered invalid and will not be reviewed.

Submission requiring full HREC review must be received by the monthly closing date. The closing dates for research submissions are listed on our website. Closing time is 12midday on the closing date.

Submissions requiring full HREC review

For submissions requiring full HREC review, please submit the following documents electronically to research.ethics@mmri.mater.org.au:

  • Cover Letter
  • One (1) electronic version of completed form Online Forms website NEAF and low and negligible risk (LNR) application forms
  • One (1) copy of the research protocol
  • One (1) copy of all supporting documents, please refer to the checklist 

    PLUS

  • Five (5) hardcopies of all documents i.e. application form and all supporting documents. This to include one (1) original, plus four (4) copies, with the following signatures:
    • Coordinating Principal Investigator (CPI)
    • Supervisor/s for the purpose of undergraduate or higher degree research

NOTE: signatures such as Head of Department and Executive Director are not required by the HREC - these are only required by the Research Governance Office.

Amendments to approved studies may be submitted at any time

Please submit the following documents electronically to research.ethics@mmri.mater.org.au + 1 hard copy of each to the HREC Office:

  • Cover Letter
  • Summary of amendments and rationale for amendments
  • One (1) tracked and one (1) clean copy of the revised research protocol, including new version number, date and document identifier in the footer
  • One (1) tracked and one (1) clean copy of other relevant documents including patient information and consent forms (PICF) 

(please also provide a copy to the Research Governance Office)

Submissions requiring low or negligible risk review

Low or Negligible Risk (LNR) Applications can be submitted at any time to research.ethics@mmri.mater.org.au.

Please email the LNR submission code to the Research Ethics Office (and cc Research Governance Office) and also provide an electronic copy of the following:

  • Cover Letter
  • Completed LNR form with the following signatures:
    • Coordinating Principal Investigator (CPI)
    • Supervisor/s for the purpose of undergraduate or higher degree research
  • Research protocol including new version number, date and document identifier in the footer.

*No hardcopies are required for LNR submissions.

 Progress reports

One (1) electronic and one (1) hardcopy of the progress report.

(please provide a copy to the Research Governance Office)

Serious adverse events report

  • One (1) electronic and one (1) hardcopy of completed SAE report

(please provide a copy to the Research Governance Office)

SUSAR and safety reports

  • One (1) electronic and one (1) hard copy of a cover letter stating the Principal Investigator has reviewed the report and what recommendations, if any, he/she makes as a result of these events.
