Participate in a study aimed at understanding how to improve health and wellbeing of older adults by preventing or slowing frailty.
Frailty is the most significant challenge to “ageing well” in Australia. The primary aim of this study is to understand if mildly frail older people living in the community who receive a supervised frailty program, including exercise, nutrition, medication review and social supports facilitated by a health coach and a web-based program, will have a different change in frailty at six months, compared to solely a self-directed, web-based program.
The secondary objective is to compare between the group's health-related quality of life, physical function and strength, nutritional status, medication optimisation and social connectedness.
A further objective is to examine the cost-effectiveness of the supervised frailty intervention compared to the self-directed program and understand the individual and contextual barriers and enablers to participating in and delivering the programs and how these influence mechanisms of impact and outcomes.

Who can take part?
For inclusion a potential participant must meet all the following criteria:
1. Received a comprehensive geriatric assessment (CGA) at a public or private Australian geriatric medicine clinic/practice in the preceding 12 months;
2. Aged ≥65 years, or ≥55 years if Aboriginal or Torres Strait Islander;
3. Living in the community (i.e., not in aged care or hospitalised);
4. Have access and ability to use a computer or other electronic device with internet service (self-report); and
5. Be able to travel to the exercise professional training sessions of their State.

What is involved for me?
A program to optimise your exercise, nutrition, medication and social support based on latest research, with check-ins at 6 and 12 months.
You will be randomly assigned to receive a self-directed program with online resources or a supported program (involving two exercise sessions each week with an exercise professional, one dietitian assessment and fortnightly/monthly health coach support) with online resources.
